Job summary

CVHS is seeking a Clinical Pharmacy Services Representative to serve as a primary contact for patients and clinic staff, supporting pharmacists in optimizing access to clinical services.

The role involves coordinating medication therapy management, assisting with prior authorizations and Medicare wellness visits, and collaborating with healthcare professionals to improve patient outcomes while maintaining efficient office operations.

Qualifications

• High School Diploma or equivalent required.
• Pharmacy Technician License preferred but not required.
• On-the-job training under supervision of a licensed pharmacist.

Responsibilities

• Serve as a point of contact for patients and clinic staff.
• Assist pharmacists and clinic staff in medication therapy management.
• Process prescription prior authorizations and specialty pharmacy assessments.
• Conduct medication history reviews and schedule Medicare Annual Wellness visits.
• Collaborate with healthcare professionals to optimize patient outcomes.
