Baptist Health Deaconess Madisonville is seeking a Pharmacy Care Coordinator to work in a specialty clinic setting. The role involves collaborating with insurers, nursing, social work, and pharmacy to obtain medication authorizations and registration with manufacturer programs where applicable.
The coordinator will assist with benefits investigations and prior authorization processes, locate copay assistance options for patients, and escalate clinical questions to the clinic specialty
Pharmacy Care Coordinator responsibilities include
Works in collaboration with insurance companies, nursing, social workers, and pharmacy to obtain appropriate authorization for medications including registration with drug specific manufacturer programs when applicable.
Performs tasks related to prescription billing including completing benefits investigations, assisting with the prior authorization process, and locating copay assistance options for patients to ensure affordability of specialty medications.
Escalates clinical support needs and questions to the clinic specialty pharmacist.
Assists with clinic intervention outcomes tracking to monitor value of services.
Coordinates patient outreach during clinic visits or via telephonic follow up to ensure continuity of care and optimize patient outcomes.
Responsibilities may include working to establish and maintain short and long-term quality improvement goals.
The scope and complexity of the position requires a diverse and highly developed skill set including but not limited to: time-management, communication, creativity, vision, teamwork, management, leadership, clinical knowledge base/skills, mentorship, change management, scholarship and teaching. Candidates must be able to work effectively with a multidisciplinary team, handling multiple responsibilities in a fast-paced environment. Physically positioned in a specialty medical clinic setting and work in direct contact with the Clinic Specialty Pharmacist.
