Qualifications

• Active, unrestricted Pharmacist license or Pharmacist Intern (Pharm D) license in Puerto Rico.
• Experience working independently within a cross functional team environment.
• Solid knowledge of drug products and clinical pharmacy.

Responsibilities

• Engage members telephonically and schedule pharmacist consultations.
• Maintain HIPAA privacy and confidentiality.
• Use prescription claims data to generate clinical recommendations.
• Assess medication regimens for unnecessary or harmful medications.
• Provide patient education on medications and disease states.
• Document patient care notes clearly and professionally.
• Keep up with clinical trends and participate in meetings.
• Meet productivity and performance expectations.
