Job summary

Pharmacy Careers is seeking a Remote Patient Care Pharmacist to provide telephonic or video-based counseling and adherence support from home. This role focuses on improving outcomes through MTM and clinical monitoring. You will collaborate with physicians and care teams, conduct reconciliations for high-risk patients, and document interventions per standards.

Qualifications

• Pharmacist education required: PharmD or equivalent.
• Active U.S. pharmacist license; multi-state licenses a plus.
• Ambulatory care, MTM, or clinical experience preferred.
• Strong communication and empathy for virtual patient care.

Responsibilities

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; suggest optimizations.
• Conduct medication reconciliations and reviews for high-risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document all interventions and communications according to standards.
• Supports population-health and disease-management programs.
