Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to perform telephonic patient consultations regarding specialty medications and disease management. The role includes medication reconciliation, drug interaction screening, and side effect guidance, with independent work and a patient-centered approach.

Qualifications

• Graduate of an ACPE accredited school of pharmacy with a PharmD or B.S. Pharmacy.
• Current, unrestricted pharmacist license in the state of residence; maintain licensure with credits.
• Two years of experience as a practicing licensed pharmacist preferred.
• Strong clinical background with specialty pharmacy knowledge desirable.

Responsibilities

• Provide telephonic patient consultations on specialty medications and disease management.
• Document consultations in the pharmacy software and ensure regulatory compliance.
• Interpret prescriptions, verify drug strength and directions, and contact providers as needed.
• Coordinate with physicians for prescription clarifications and transfers; promote generics when appropriate.
• Stay current with specialty therapies and support quality and safety initiatives.
• Back up peers and train junior pharmacists; report medication errors per policy.
