Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to join our team. This role focuses on telephonic patient consultations regarding speciality medications, medication reconciliation, and disease state management.

You will ensure accuracy, compliance, and timely processing of orders while maintaining high-quality patient care. The position offers a comprehensive benefits package and an annual bonus of 7.5%.

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree.
• Current, unrestricted pharmacist license in state of residence; must continually obtain education credits to maintain active licensure.
• At least two years of experience as a practicing licensed Pharmacist preferred.

Responsibilities

• Provide patient education via telephonic consultations for patients of the specialty pharmacy, focusing on disease state management and medication use.
• Provide detailed clinical documentation of patient consultations and interventions in pharmacy software per internal policies and external regulatory bodies.
• Interpret and clinically evaluate prescriptions for specialty medications; contact providers/patients to verify prescription details to expedite processing.
• Contact physicians for prescriptions, consult with physicians/nurses on questions, and coordinate prescription transfers; convert to generics when possible.
• Verify prescription information entered in the system; answer clinical questions from the pharmacy team.
• Adapt to new workflows, stay current with specialty therapies and new drug approvals and treatment guidelines.
• Back up other pharmacists as needed and train less experienced staff.
• Document and report medication errors per policies; participate in error reduction and ISMP standards.
• Operate in compliance with federal and state laws and high ethical standards.
• Other duties as assigned.
