Carle Health is seeking a hospital pharmacist to implement pharmaceutical services across the Medical Center, compounding and dispensing medications and performing clinical duties like therapeutic drug monitoring and patient-appropriate dosing.

The role requires a licensed pharmacist and a B.Pharm; the pharmacist will check medications, verify orders, enter into HBOC, manage IV admixtures, and communicate with healthcare professionals.

Qualifications

  • Licensed Pharmacist with valid license.
  • Bachelor’s Degree in Pharmacy.

Responsibilities

  • ACCEPTS RESPONSIBILITY FOR CHECKING ALL MEDICATIONS LEAVING THE DEPARTMENT TO VERIFY QUALITY ASSURANCE.
  • The pharmacist must have a working knowledge of chemotherapy agents.
  • The pharmacist must have a working knowledge of hyperalimentation.
  • The pharmacist must have a working knowledge of IV admixture.
  • The pharmacist must have a working knowledge of narcotic control.
  • The pharmacist must have a working knowledge of investigational drugs.
  • The pharmacist must have a working knowledge of therapeutic drug monitoring.
  • The pharmacist must have a working knowledge of medication storage and dispensing guidelines.
  • DISPENSES MEDICATIONS FROM PHYSICIAN ORDERS.
  • Ensuring accurate entry of the order into the computer to furnish hospital personnel with current medication administration reports.
  • Accepting responsibility for verifying dosages on the physician order are patient appropriate.
  • The pharmacist dispenses medications per policy and procedure in a timely manner, using reference materials when necessary to ensure the accuracy and appropriateness of patient care.
  • COMPETENT IN COMMUNICATING TO HEALTHCARE PROFESSIONALS REGARDING AN ORDER.
  • The pharmacist works with healthcare professionals to provide the most appropriate and cost-effective drug therapy for each individual patient.
  • The pharmacist communicates with healthcare professionals regarding medication orders.
  • The pharmacist plays an active role in planning new functions and updating current services provided by pharmacy to assure high standards of pharmacy practice in the Medical Center.
  • The pharmacist participates in in-service programs relating to new procedures, policies, equipment and treatments.
  • The pharmacist participates in projects in the department, or outside the department, which may be formally assigned.
  • COMPETENT IN WORKING WITH VARIOUS COMPUTER SYSTEMS, WRITING IN PATIENT CHARTS, DOSING MEDICATIONS BASED ON PATIENT SPECIFIC PARAMETERS, AND OTHER CLINICAL FUNCTIONS THAT MAY ARISE.
  • The pharmacist enters oral medications into HBOC System.
  • The pharmacist enters IV medications into HBOC System.
  • The pharmacist oversees technician in operating Omnicell System.
  • The pharmacist is able to dose all drugs in Pharmacokinetic Dosing Service.
  • The pharmacist monitors for all adverse drug reactions on patients.
