Qualifications

• Active Clinical Pharmacist license in Illinois (IDFPR).
• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy Degree.
• Preferred: Omnicell administration and automation experience.
• Preferred: Pharmacy billing and medication set up experience.
• Preferred: Residency or equivalent hospital pharmacy experience.

Responsibilities

• Interpret medication orders for safety and proper use; resolve related problems.
• Monitor therapy for drug interactions; intervene when necessary.
• Evaluate and document appropriate medication choices for outcomes.
• Consult on pharmacokinetics and adverse reactions with staff.
• Process medication orders into the Electronic Health Record accurately.
• Maintain comprehensive patient pharmacy profiles with key data.
• Oversee packaging, labeling, distribution; monitor controlled substances.
• Dispense pharmaceuticals including sterile, chemo, and parenteral nutrition.
