Job summary

CommunityCare is seeking a Clinical Pharmacist - Population Health to optimize medication safety, effectiveness, adherence, and affordability. You will work with members, providers, and interdisciplinary teams to review medications, support chronic condition management, and lead PHM opioid safety initiatives.

Qualifications

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited School of Pharmacy.
• Active, unrestricted pharmacist license in Oklahoma, or ability to obtain licensure within 180 days of hire.
• Minimum five years of clinical pharmacy experience in managed care/population health.
• Experience with Medicare Advantage/DSNP and transitions of care preferred.
• Pharmacy residency or board certification preferred.

Responsibilities

• Conduct medication reviews to identify safety concerns and optimize therapy.
• Recommend evidence-based, cost-conscious interventions and document outcomes.
• Reconcile medications after hospitalizations and transitions of care.
• Participate in interdisciplinary care planning for safe medication management.
• Educate members and caregivers on medication use, safety, and affordability barriers.
• Oversee PHM opioid safety program and high-risk med list.
• Perform clinical reviews of specialty/high-cost meds considering patient factors.
• Review dosing regimens to improve outcomes and minimize waste.
• Identify safe, cost-effective therapeutic alternatives and dosing strategies.
• Collaborate with behavioral health and other clinicians on complex needs.
• Support Pharmacy Case Management with targeted reviews.
• Address coverage/formulary barriers with Pharmacy and Utilization Mgmt teams.
• Develop pharmacy workflows, clinical guidance, and staff training.
• Monitor intervention outcomes, utilization, and cost trends; support quality/regulatory readiness.
