Mercy Hospital Southeast is seeking a part-time Clinical Pharmacist to join the healthcare team. You will collaborate with physicians, nurses, and other professionals to ensure safe and effective medication therapy and serve as a resource for drug information.

Responsibilities include reviewing patient records, developing optimized medication plans, and participating in Medication Therapy Services. Mercy emphasizes mission-driven, patient-centered care and professional growth.

Formación

• Education BS in Pharmacy or PharmD (required).
• Current pharmacist license in the applicable state (required).
• Two+ years of clinical pharmacy experience preferred.
• PGY‑1 Pharmacy Residency preferred.

Responsabilidades

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to ensure treatment plans address all clinical needs.
• Develop medication plans that minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications, dosing, and therapy recommendations.
• Provide guidance on proper drug administration and assess treatment outcomes.
• Serve as a clinical resource for care teams and maintain subspecialty expertise as appropriate.
• Participate in Medication Therapy Services and patient assessments.

BS in Pharmacy

Doctor of Pharmacy (PharmD)

PGY‑1 Pharmacy Residency (preferred)

Current pharmacist license
