Job summary

Northwestern Medicine seeks a Clinical Pharmacist in Illinois to ensure safe and effective medication use across the hospital. You will provide information to clinicians and patients, monitor therapy, and document in the EHR to support integrated pharmacy services.

The position emphasizes collaboration with nursing and prescribers, pharmacokinetics consulting, and participation in quality improvement and medication use reviews to optimize patient care.

Qualifications

• Active Clinical Pharmacist license in the state of Illinois (IDFPR).
• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy Degree.

Responsibilities

• Interprets medication orders to ensure safety and proper use of medications and coordinates the solving of medication related problems between pharmacy, nursing, and prescribers.
• Monitors medication therapy for significant drug interactions and intervenes as needed.
• Evaluates and documents that the medication(s) selected are appropriate for achieving the desired therapeutic outcome and communicates issues to the prescriber.
• Acts as a consultant in pharmacokinetics and adverse reactions to physicians, nurses and other healthcare professionals.
• Processes all medication order information into the Electronic Health Record to ensure accuracy of records.
• Maintains a comprehensive pharmacy patient profile and ensures documentation and compliance.
