Bon Secours Mercy Health is seeking a Staff Clinical Pharmacist at West Hospital in Ohio. The role focuses on safe, rational, and cost-effective therapy and requires adherence to federal, state, and accrediting standards.

The pharmacist participates in clinical programs 50-90% of the time and collaborates with care teams to optimize outcomes. Responsibilities include medication management, reviews, education, and precepting students.

Formación

• Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD (required)
• Advanced pharmacy degree (Doctor of Pharmacy, Masters, Residency) (preferred)
• Maintains current license for pharmacist in the state(s) where practicing (required)
• Pharmacy residency or 3 years equivalent experience; BCPS or other certification preferred

Responsabilidades

• Medication management for a specific clinical area or patient population
• Participate in clinical programs to improve patient care, optimize medication therapy and promote health
• Perform medication reviews to ensure safe and appropriate use of medications and achieve defined patient care outcomes
• Maintain proficiency in order processing and support pharmacy operations in medication distribution
• Engage in cost-containment efforts towards medication utilization and best use of resources
• Collaborate with pharmacy team members and other health care professionals to deliver high-quality services
• Identify, document and resolve adverse medication events; evaluate data for prevention
• Provide medication education to improve patient/associate understanding and compliance
• Serve as a preceptor for students and residents
• Process orders and provide medication review for appropriateness and safety
