Mercy is seeking a clinically driven Pharmacist to join our team and optimize evidence-based medication therapy across care settings.

You will partner with physicians, nurses, and other healthcare professionals to improve patient outcomes while serving as a trusted medication resource. This role emphasizes education, leadership, and collaboration within Mercy’s mission-driven environment.

Qualifikationen

• BS in Pharmacy or PharmD from an accredited school.
• Current pharmacist license in the state of practice.
• Strong clinical judgment and patient-centered care.
• Ability to work effectively in a fast-paced healthcare environment.

Aufgaben

• Review patient records and med orders to ensure safe, effective therapy.
• Develop individualized medication treatment plans.
• Monitor outcomes and adjust recommendations as needed.
• Identify and prevent adverse drug events and interactions.
• Provide dosing guidance and formulary recommendations.
• Consult with physicians, nurses and patients on therapies.
