Job summary

Mercy Health seeks a Clinical Pharmacist to participate in clinical programs 50-90% of the time, ensuring safe, rational, and cost-effective therapy. You will maintain compliance with federal, state, and accrediting standards while collaborating with teams to improve patient care.

The role requires PharmD (or BS) from ACPE-accredited school, active state licensure, and potential involvement as a preceptor for students and residents. Immunization certification preferred; ACLS optional.

Qualifications

  • Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD (required).
  • Advanced pharmacy degree – Doctor of Pharmacy, Masters, Residency (preferred).
  • Maintains current pharmacist license in the practicing state.
  • Immunization certified (preferred).
  • Pharmacy residency or 3 years equivalent experience; BCPS or similar (preferred).

Responsibilities

  • Participate in medication management activities for a clinical area or patient population.
  • Perform medication reviews and therapy management to ensure safe, effective use.
  • Support operations to ensure timely medication availability.
  • Engage in cost-containment efforts for medications and pharmacy resources.
  • Educate patients, staff, and students; serve as a preceptor as assigned.
  • Collaborate with health care teams to improve patient care and outcomes.
  • Document and report adverse events; utilize data for trending and prevention.
  • Process orders and provide clinical support during medication reviews.
