Job summary

Qualifications

• Licensed in the state(s) of practice or eligible for reciprocity
• Immunization and non-vaccine injectable certification
• Credentialing and privileging required
• Bachelor's or Doctorate degree in pharmacy from ACPE-accredited school or NABP FPGEC
• No experience required
• Must provide age-appropriate patient care knowledge

Responsibilities

• Oversees pharmaceutical care, including literature review and regimen design
• Counsels patients and caregivers on medication therapy and safety
• Designs and monitors patient-specific pharmacotherapeutic regimens
• Reviews medication orders and prescriptions
• Participates in medical emergencies when applicable
• Supports medication safety and quality improvement activities
• Manages formulary adherence and medication-use processes
• Assists in acquiring non-formulary, compassionate use, and investigational meds
• Ensures compliance with policies, procedures and regulations
• Prepares, dispenses, and oversees storage/handling of medications
• Serves as preceptor and mentors pharmacy teammates and students
• Directs and checks the work of pharmacy support staff to ensure safety
