Job summary

AbsoluteCARE Medical Center & Pharmacy is seeking a Clinical Pharmacist to support member-centered care. You will round with provider teams, supervise technicians, and conduct medication reconciliation to optimize outcomes.

You will lead MTM activities, educate patients, manage inventory, and coordinate packaging and delivery while supporting population health initiatives and staff in-services. This role requires PharmD or license and 2–3 years of related experience.

Qualifications

• PharmD or licensed pharmacist.
• 2–3 years’ experience in a similar role.
• Demonstrated passion to improve healthcare delivery and work with diverse communities.
• Ability to build and maintain relationships with a diverse population.
• Strong teamwork and customer service orientation.

Responsibilities

• Rounding with provider teams to support patient care.
• Conduct medication reconciliation and MTM to optimize outcomes.
• Supervise pharmacy technicians and oversee accuracy of fills.
• Manage inventory, packaging, and delivery coordination.
• Support population health initiatives (e.g., HIV and CKD) and staff training.
