Job summary

Community Care Cooperative is seeking a Pharmacist I-III to oversee medication distribution and collaborate with health center physicians on drug therapy choices. You will manage inventory, counsel patients, and help ensure safe medication use across our clinics.

The role includes supervising technicians, maintaining records for controlled substances, and participating in clinical programs to optimize pharmacotherapy and patient safety within a compliant, patient-centered environment.

Qualifications

• Graduate degree from ACPE-accredited pharmacy college.
• Valid state pharmacist license.
• CPR or CPR/AED certification.
• MTM certification preferred.
• Experience in pharmacy, retail, medical, or customer service.

Responsibilities

• Provides pharmacy information to health care professionals and counsels patients on drug therapies.
• Develops clinic staff's pharmacy knowledge through clinical programs and training.
• Performs medication reconciliation and reviews based on patient needs.
• Identifies barriers to adherence and develops plans to improve med-taking behavior.
• Complies with state and federal drug laws and regulations.
• Maintains records for controlled substances and removes outdated drugs.
• Maintains 340B inventory as appropriate.
• Supervises support personnel and ensures proper labeling and order accuracy.
• Maintains current registration and infection-control protocols.
• Ensures safe and clean working environment; supports team objectives.
