Job summary

AbsoluteCare seeks a Clinical Pharmacist to support high-quality, member-centered care. You will round with provider teams, supervise technicians, perform medication reconciliation, and educate patients on medications and adherence.

The role also supports population health initiatives such as HIV and CKD, manages inventory and deliveries, and mentors pharmacy students. Requires PharmD or RPh and 2–3 years of experience; compensation is $140,000–$150,000 per year.

Qualifications

• Pharm D or registered pharmacist.
• 2-3 years of experience in a similar role.

Responsibilities

• Rounding with provider teams to support coordinated care.
• Supervise pharmacy technicians and ensure accurate dispensing.
• Conduct medication reconciliation and MTM/CMR services.
• Educate patients on medications and adherence strategies.
• Coordinate medication packaging, delivery, and inventory management.
• Support population health initiatives (HIV, CKD) and staff training.
