Job summary

Advocate Health is seeking an experienced Pharmacist to oversee pharmaceutical care within their clinical practice, ensuring safe and effective medication therapy in line with area- and region-specific standards. You will educate patients and caregivers, review medication orders, and participate in emergencies while supporting safety and quality initiatives.

The role includes mentoring pharmacy students and staff.

Qualifications

• Pharmacy degree from ACPE-accredited school or NABP FPGEC certificate required.
• Licensed in the state of Georgia or eligible for reciprocity.
• Hospital experience and PGY-1 in hospital practice preferred.

Responsibilities

• Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Provides counseling to patients and caregivers on medication therapy, adverse effects, compliance, and proper use.
• Designs, monitors, and evaluates patient-specific pharmacotherapeutic regimens and implements recommendations per policies.
• Reviews and interprets medication orders and prescriptions accurately.
• Participates in medical emergencies when applicable.
• Supports medication safety and quality improvement activities; reports events through appropriate channels.
• Performs quality assurance and performance improvement of the medication-use system.
• Collaborates on formulary adherence and medication-use process; assists with non-formulary and investigational medications.
