The role requires 1–2 years of hospital patient care experience, with PGY-1 preferred, and licensure as an RPH with possible board certification consideration.

Qualifications

• Doctoral degree in Pharmacy required or equivalent from an accredited college of Pharmacy.

Responsabilités

• Evaluates medication orders for accuracy and dispenses medications as per orders and procedures.
• Assesses feasibility of compounding extemporaneous products and recommends alternatives.
• Prepares IV admixtures and sterile products including large volume parenteral and hazardous agents.
• Responds to drug information requests.
• Monitors and optimizes drug therapy under supervision for safety and efficacy.
• Reviews patient profiles for contraindications and appropriateness of therapy.
