Job summary

Qualifications

• Doctoral degree in Pharmacy or equivalent from an accredited college of Pharmacy.
• PGY-1 preferred.

Responsibilities

• Evaluates medication orders for accuracy and thoroughness; resolves discrepancies with physicians, pharmacists or nursing staff.
• Assesses feasibility of compounding extemporaneous products and recommends alternatives.
• Prepares IV admixtures and sterile products, including hazardous agents.
• Responds to drug information requests.
• Monitors and optimizes patient drug therapy under supervision.
• Reviews patient profiles for contraindications and appropriate dosing.
