Job summary

Qualifications

• Doctoral degree PharmD from an accredited college of Pharmacy required or an equivalent Pharmacy degree from an accredited college of Pharmacy required.
• 1-2 years of direct patient care experience in a hospital setting required.
• PGY-1 preferred.
• RPH - Registered Pharmacist Licensure in the state of practice upon hire required.
• Board certification upon hire preferred.

Responsibilities

• Evaluates medication orders for accuracy and dispenses medication.
• Assesses feasibility, compounds, or verifies extemporaneous products and recommends alternatives.
• Prepares IV admixtures and sterile products including large volume parenteral, mini-bags, TPN solutions and hazardous agents.
• Independently responds to non-evaluative drug information requests.
• Monitors and optimizes patient drug therapy under supervision of a senior pharmacist/manager.
• Reviews patient profiles for contraindications and appropriate dosing; screens drugs/dosages.
• Dispenses controlled substances and maintains perpetual inventory; ensures legal and policy compliance.
• Provides supervision of support personnel and oversees workflow to ensure efficient operations.
• Enters and verifies computer entries; may counsel patients on safe use and adverse effects.
• Keeps knowledge current through literature, seminars, conferences and continuing education.
