Job summary

Tria Health seeks a pharmacist to deliver high-quality clinical pharmacy services and MTM to members and providers. You will educate members, evaluate drug therapy, and document outcomes while collaborating with other healthcare professionals.

The role supports a hybrid work schedule after training and requires licensure in TX or home state with priority on hospital/managed care MTM experience.

Qualifications

• Pharm D required; licensure in good standing.
• Texas license and license in home state required or within 90 days of hire.
• Residency or Fellowship highly desired ; BCPS/CGP preferred.
• 4 years MTM or medication therapy management experience preferred.

Responsibilities

• Serve as a clinical pharmacy resource for members, clients, brokers, Third Party Administrators, peers, and coworkers.
• Provide education and consultations to eligible members of the Tria Health program.
• Identify and plan for resolution of drug-related problems.
• Evaluate appropriateness, effectiveness, safety, and adherence of drug therapy.
• Document identified problems and assess outcomes of medication therapy.
• Communicate with other healthcare providers.
• Provide continuity of care to the patient load assigned.
• Develop and maintain clinical programs.
