Legacy Community Health in Houston, TX, invites a Pharmacist to join a collaborative, multidisciplinary team. You will process prescriptions, counsel patients, and support safe, effective medication use within a community health setting.

The role emphasizes MTM, drug interaction reviews, and close collaboration with providers to improve health outcomes. Eligible Pharmacist-in-Charge duties include leadership responsibilities with stipends.

Qualifications

• Pharmacist degree from an accredited program and licensure in Texas.
• Immunization certification is required.
• Active Texas Pharmacist license at hire.
• Active or in-process Texas Pharmacist Preceptor license.
• Maintain First Aid/CPR certification; AAHIVP preferred or pursued within 18 months.

Responsabilités

• Interpret, clarify, and transcribe medication orders accurately.
• Review prescriptions for appropriate drug selection and dosage.
• Dispense medications and maintain inventory per regulations.
• Provide MTM and patient counseling to optimize outcomes.
• Ensure regulatory compliance and oversee pharmacy staff.
