Qualifikationen

• PharmD and valid pharmacist license in good standing.
• Residency and/or Fellowship highly desired.
• BCPS/CGP or Certified Disease Educator certification desirable.
• 1–3 years hospital, managed care, or consultant experience with chronic therapy management.
• Doctor of Pharmacy degree with related medication therapy management experience.
• Texas license required; if not held, plan to obtain within 90 days of hire.
• Professional liability insurance is required.

Aufgaben

• Serve as a clinical pharmacy resource for members, clients, brokers, and coworkers.
• Provide education and consultations to Tria Health program participants.
• Identify and resolve drug-related problems and assess outcomes of therapy.
• Evaluate appropriateness, safety, and adherence of drug therapy.
• Document issues and communicate with other healthcare providers.
• Develop and maintain clinical programs and support APPE rotations.
• Answer Help Desk calls and balance workload across departments.
