Qualifications

  • PharmD and valid pharmacist license in good standing.
  • Residency and/or Fellowship highly desired.
  • BCPS/CGP or Certified Disease Educator certification desirable.
  • 1–3 years hospital, managed care, or consultant experience with chronic therapy management.
  • Doctor of Pharmacy degree with related medication therapy management experience.
  • Texas license required; if not held, plan to obtain within 90 days of hire.
  • Professional liability insurance is required.

Responsibilities

Description

The ideal candidate will provide high-quality clinical pharmacy services to patients and their health care providers by upholding and embodying the company’s mission statement.

Essential Duties and Responsibilities:
  • Serve as a clinical pharmacy resource for members, clients, brokers, Third Party Administrators, peers, and coworkers.
  • Provide education and consultations to eligible members of the Tria Health program.
    • Identify and plan for resolution of drug-related problems.
    • Evaluate appropriateness, effectiveness, safety, and adherence of drug therapy.
    • Document identified problems and assess outcomes of medication therapy.
    • Communicate with other healthcare providers.
  • Provide continuity of care to the patient load assigned.
  • Develop and maintain clinical programs.
  • Support the advanced pharmacy practice experience (APPE) rotation site.
  • Completion of annual continuous professional development plan.
  • Assist with training of coworkers from other departments upon request.
  • Answer incoming Help Desk calls.
  • Work in other functional areas to cover absences or relief, to equalize peak work periods or otherwise to balance the workload.
  • Other duties as assigned.

Requirements
Education/Experience:
  • Required: Pharm D, Registered Pharmacist (license in good standing)
  • Highly Desired: Residency and/or Fellowship
  • Highly Desired (any of the following or a desire to pursue): BCPS, CGP, Certified Disease Educator, Certified Disease Manager
  • Prefer: 1-3 years of experience in a hospital, managed care or consultant setting with direct patient interaction via provision of chronic drug therapy management or MTM.
  • Must possess a Doctor of Pharmacy degree and work-related experience in medication therapy management, or Bachelor of Science degree in pharmacy with four years of work-related experience in medication therapy management.
  • Exposure to clinical and analytical problem solving and expertise in drug information is preferred.
  • Pharmacist license for Texas and the state you reside in are required. If you do not carry a Texas license hiring will be contingent on attaining license within 90 days of hire date.
  • Professional liability insurance is required.
Job Competencies:
  • Read, analyze, and interpret general business periodicals, professional journals, technical procedures, governmental regulations, and clinical literature.
  • Write reports, business correspondence, and procedure manuals.
  • Effective communication skills are required to succeed in this position.
  • Work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry.
  • Apply concepts such as fractions, percentages, ratios, and proportions to practical situations is desired.
  • Solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists is desired.
  • Interpret a variety of instructions furnished in written, oral, diagram, or scheduled form.
  • Intermediate to advanced knowledge of Word, Access, Excel, Database software, E-mail, and Internet

Individual must occasionally lift and/or move up to 5 pounds.
  • Work Environment: This job operates in a professional office environment where standard office equipment such as computers, phones, copiers, filing cabinets, and printers are utilized. The noise level in the work environment is usually minimal.
  • Location: Tria Health is in Kansas City, MO (Crossroads area). A hybrid schedule is offered after training.
  • Schedule: 40-hour work week - 3 evening shifts and 2-day shifts/week. 1 Saturday day shift per month in place of an evening shift that week.
