Job summary

Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe, appropriate pharmacy services in a home care setting. You will coordinate patient services with Nursing, monitor progress toward goals and educate patients, caregivers and staff on medication use.

The role requires graduation from an accredited School of Pharmacy and an active state license, with options for IV admixture experience or ASHP-related training as preferred qualifications.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: at least 1 year of sterile IV admixture experience or related advanced pharmacy training.

Responsibilities

• Determine patient suitability for home care and gather patient information.
• Design, monitor and adjust pharmacy care plans for home care patients.
• Provide medication-use education to staff, patients and caregivers.
• Ensure continuity of pharmaceutical care across home and other settings.
• Prepare and dispense medications following policies and ensure proper labeling.
