Job summary

Option Care Health is seeking a Clinical Pharmacist to provide safe pharmacy services in the home infusion setting. This entry-level role coordinates with Nursing, educates patients and caregivers, and monitors patient progress toward goals.

The position requires a Pharmacist license and graduation from an accredited School of Pharmacy. Experience in sterile IV admixture, MTM or residency is preferred. A comprehensive benefits package is offered.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license.
• One of: sterile IV admixture, non-sterile compounding, MTM or residency.

Responsibilities

• Assess patient suitability for home care and coordinate services.
• Design and monitor patient-specific pharmacy care plans.
• Provide medication-use education to staff, patients and caregivers.
• Ensure continuity of pharmaceutical care across home and other settings.
• Adhere to safety, regulatory, and quality standards.
