Job summary

Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services for home care patients. The role coordinates with the Nursing Department, monitors patient progress, and educates patients, caregivers, staff and professionals on medication use.

As a licensed pharmacist, you will design and monitor patient-specific pharmacotherapy plans, modify therapy as needed, and ensure continuity of pharmaceutical care across settings in a rapidly evolving

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy
• At least 1 year of experience in sterile IV admixture or related areas or equivalent

Responsibilities

• Assess patient suitability for home care per policy.
• Design and monitor patient-specific pharmacotherapy plans.
• Modify care plans based on monitoring data and lab values.
• Educate staff, patients and caregivers on medication use.
• Ensure continuity of pharmaceutical care across settings.
• Prepare and dispense medications following policies.
• Maintain equipment and inventory accuracy for narcotics.
• Respond to drug information requests timely.
