Job summary

Option Care Health is hiring an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services in home care. You will coordinate with Nursing, monitor patient progress, and educate patients, caregivers and staff on medication use.

The role requires a licensed pharmacist with education from an accredited school and experience in sterile IV admixture, MTM, or related residencies. Travel and supervisory duties may be included depending on site needs.

Qualifications

• Pharmacist graduate with active license in good standing.
• Experience in home care, hospital or clean room setting is preferred.
• Wamiliarity with IV admixture and MTM is a plus.

Responsibilities

• Assess patient eligibility for home care and collect necessary information.
• Design and monitor patient-specific pharmacotherapy plans.
• Educate patients, caregivers, and staff on medication use.
• Ensure continuity of pharmaceutical care across settings.
• Maintain safe medication handling and accurate dispensing.
