Job summary

Option Care Health, the nation’s leading home infusion provider, seeks an entry-level Clinical Pharmacist to deliver safe and appropriate pharmacy services in the alternate setting. You will coordinate with the Nursing Department, monitor patient progress, and educate patients, caregivers, staff and other professionals on proper medication use.

Graduates must hold an accredited pharmacy degree with an active license.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: at least one year of sterile IV admixture procedures, non-sterile compounding, immunizations, MTM, or equivalent residency/experience.

Responsibilities

• Determine suitability of patients for home care and collect patient-specific information.
• Design, monitor, and evaluate patient-specific pharmacy care plans for pharmacotherapy.
• Provide medication-use education to Option Care staff, patients, and caregivers.
• Ensure compliance with state/federal laws, DEA regulations, and USP standards.
• Maintain continuity of pharmaceutical care across home, hospital, and other settings.
