Job summary

Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services in the home care setting. The role coordinates with Nursing, educates patients and caregivers, and ensures safe medication use according to policy and regulatory standards.

You will participate in multidisciplinary teams, manage medication therapies, and support continuous quality improvement while operating within a supportive, clinical culture that values patient-centered care.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: sterile IV admixture, non-sterile compounding, immunizations, MTM, residency, or equivalent experience.

Responsibilities

• Determines suitability of patients for home care and collects patient information.
• Designs and monitors patient-specific pharmacy care plans for home care.
• Modifies care plans based on monitoring data; assesses lab values.
• Provides medication-use education to staff, patients and caregivers.
• Ensures continuity of pharmaceutical care between home and other settings; prepares and dispenses medications.
