Job summary

Option Care Health is seeking a licensed Clinical Pharmacist to provide safe, appropriate pharmacy services in the home care setting. The role is entry-level in clinical pharmacy, coordinating with Nursing and educating patients and caregivers on medications.

The position requires graduation from an accredited School of Pharmacy, active state licence, and experience in sterile IV admixture, MTM, or related areas. Travel is minimal and compliance with regulations is required.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy licence as granted by the State Board of Pharmacy.
• One of: sterile IV admixture, non-sterile compounding, MTM, residency, or equivalent experience.

Responsibilities

• Assesses patients' suitability for home care and collects necessary information.
• Designs and monitors patient-specific pharmacy care plans for home care.
• Provides medication-use education to staff, patients and caregivers.
• Ensures continuity of pharmaceutical care across home and other settings.
