Job summary

LiveWell Homecare Agency, LLC is seeking a licensed pharmacist to provide inpatient hospital pharmacy services, practicing at top of licensure through MTMS protocols that support clinical intervention and medication optimization.

You will assess, monitor, initiate, and adjust medication use for diverse patient populations, supervise pharmacy staff, and coordinate with care teams to ensure safe dispensing and smooth transitions.

Qualifications

• Bachelor’s Degree in Pharmacy required.
• Minimum 2 years of experience; supervisor experience not required.
• MO MTM license (Medication Therapy Services) required.
• Doctorate in Pharmacy preferred; 2–5 years experience preferred.

Responsibilities

• Manage workflow and supervise pharmacy staff, interns, and volunteers.
• Provide drug information, staff training, and support medication therapy activities.
• Evaluate medication orders for safety and efficacy, documenting all actions.
• Identify opportunities to improve efficiency and safety in medication use.
