Job summary

Aurora Health Care is seeking a field-based supervisor to drive quality patient care across home health operations. You will lead a team, ensure chart audits are completed, and act on identified trends to support operational goals.

The role emphasizes staff development, on-going education, and collaboration with managers to improve processes and maintain regulatory compliance. Travel is required.

Qualifications

• Bachelor’s degree in Nursing or related field.
• Valid driver's license with a registered, insurable car.
• 5 years of clinical nursing or rehabilitation experience.
• 1 year of supervisory experience preferred (staff/budgets).
• Strong knowledge of home health or hospice regulations and reimbursement.
• Excellent verbal and written communication skills.
• Ability to organize, plan and delegate effectively.

Responsibilities

• Promotes an environment conducive to quality patient care.
• Assist with chart audits and ensure action is taken.
• Identify trends in audits and report results to Manager and team.
• Ensure field supervisory visits are completed according to policy.
• Provide direction to staff for coordination of resources.
• Monitor staff productivity against targets daily.
• Correct charting deficiencies impacting revenue and operations.
• Monitor clinical assessments, outcomes, and on-call utilization; advise leadership.
• Participate in staff orientation, coaching, and on-going precepting.
• Assess educational needs and facilitate in-services for staff.
• Coordinate 24-hour availability of field staff and on-call rotation.
• Conduct tracers and joint visits with ongoing feedback.
• Collaborate with manager on process improvement.
• Ensure timely OASIS data cycle time.
• Work with operations to grow and maintain patient/referral relationships.
• Maintain quality standards and evaluate workflows.
• Develop relationships with customers, physicians, and Advocate Aurora Health at Home.
