Job summary

Fairview Health Services seeks a Hematology/Oncology Pharmacist based at Southdale Cancer Center to provide comprehensive clinical pharmacy services in a complex infusion setting, focusing on oncology and specialty therapies. You will collaborate with an interdisciplinary care team to optimize medication use and patient outcomes.

The pharmacist serves as a clinical resource for patients, providers, and staff, supports formulary management and quality initiatives, and participates in education

Qualifications

• PharmD degree required and Minnesota licensure or eligibility.
• At least 1 year of pharmacist experience in complex operation or completion of an ASHP PGY1 residency.
• Experience or familiarity with oncology/infusion pharmacy preferred.

Responsibilities

• Provide advanced clinical pharmacy services for oncology and specialty infusion patients, including daily order verification and therapy monitoring.
• Act as a clinical resource, offering consultative support to providers, nurses, and teams on pharmacotherapy and safety.
• Ensure appropriate medication selection, dosing, and monitoring per evidence-based guidelines.
• Oversee sterile compounding and hazardous medication verification per USP standards and institutional policies.
• Contribute to quality improvement, formulary management, and learner education.
• Perform staff pharmacist duties as needed to ensure pharmacy operations.
• Support continuity of care across infusion sites and participate in weekend/holiday rotations.
