Qualifications

• Graduation from an accredited Nursing program and current New York State RN license.
• Bachelor’s Degree in Nursing (BSN) or higher; ADN with BSN progression may be considered.
• Current Basic Life Support (BLS) certification; Advanced certifications (NRP, ACLS, PALS) as listed.

Responsibilities

• Delivers patient care by applying the nursing process within the Relationship Centered Care model.
• Assess and evaluate patient needs and apply critical thinking in care management.
• Collaborate with the interdisciplinary team in plan of care development and evaluation.
• Prioritize all aspects of patient care including teaching, rounds, and discharge planning.
• Educate patients and caregivers about their plan of care, transitions, and health promotion.
• Document accurately and timely in compliance with hospital standards.
