Job summary

PAM Health Rehabilitation Hospital of Victoria is seeking a Clinical Navigator to be the face of the facility. You will cultivate referral relationships, meet potential patients, and work with admissions and nursing staff to move patients through the rehab process.

The ideal candidate holds a health-field degree with a current nursing or respiratory therapy license and at least two years in the designated field, preferably with LTACH rehab and marketing experience.

Qualifications

• Degree from an accredited college in a health field; nursing preferred.
• Current licensure in nursing or respiratory therapy is required.
• Prior marketing and rehab/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships, manages territory, and completes patient assessments.
• Makes sales contacts and analyzes referral/admission data from assigned territory.
• Works with admissions, nursing and staff to facilitate referral conversion.
• Provides feedback for program development and quality improvement related to customer service and satisfaction.
