PAM Health Rehabilitation Hospital of Westminster is seeking a Clinical Navigator to cultivate referral relationships, manage an assigned territory, and perform patient assessments. You will work with the Admissions Manager and nursing staff to streamline referrals and admissions, improving patient outcomes and satisfaction.

The role requires a health-related degree and current licensure in a clinical field, with marketing or LTACH experience preferred.

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts.
• Collaborates with admissions and nursing staff to improve referral and admission processes and patient satisfaction.
