Job summary

PAM Health is seeking a Clinical Navigator to cultivate referral relationships, manage territory, and complete patient assessments. You will work with the Admissions team and clinical staff to optimize the referral and admission process, ensuring high-quality patient experience.

You will collaborate with physicians, nurses, discharge planners, and payers, providing feedback to drive program development and patient satisfaction.

Qualifications

• Degree in a health-related field from an accredited college or university
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech-language pathology
• Marketing and/or rehabilitation/LTACH experience preferred

Responsibilities

• Cultivates referral relationships and manages assigned territory
• Completes patient assessments and makes sales contacts
• Analyzes referral and admission data to improve processes
• Collaborates with Admissions Manager, nursing and staff to facilitate referrals
• Builds relationships with patients, families, physicians, nurses, discharge planners, case managers and payer representatives
• Provides feedback for program development and quality improvement initiatives
