PAM Health Rehabilitation Hospital of Kyle seeks a Clinical Navigator to serve as the face of our hospital, cultivating referral relationships and guiding patients through the admission process. You will collaborate with the Admissions Manager, nursing staff, and physicians to optimize referrals and ensure a smooth patient journey from referral to discharge.

Qualifications include a health-related degree and current licensure in a listed field; two years' experience in rehab or marketing is

Qualifikationen

• Education and Training: Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Aufgaben

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts and analyzes referral and admission data from the territory.
• Collaborates with Admissions Manager and admission staff, nursing and other internal and external staff to facilitate referral conversion.
• Provides feedback and recommendations for program development and quality improvement initiatives related to customer service, the referral and admission process and patient and referral source satisfaction.
• Develops and maintains excellent relationships with stakeholders including prospective patients, family members, physicians, ICU and floor nurses, discharge planners, case managers and payer representatives.
