PAM Health Rehabilitation Hospital of Round Rock is seeking a Clinical Navigator to cultivate referral relationships, manage a territory, and assess patients. You will work with the admissions team and clinical staff to optimize referrals and admissions, while supporting program improvements and stakeholder satisfaction.

The role requires a health-related degree and current licensure, with prior rehabilitation/LTACH and marketing experience preferred.

Qualificações

• Degree in a health-related field from an accredited college/university.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech-language pathology is required.
• Two+ years in rehabilitation/LTACH and marketing experience preferred.

Responsabilidades

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts; analyzes referral/admission data.
• Collaborates with Admissions Manager, nursing, and staff to facilitate referral conversions.
• Develops feedback for program development and quality improvement related to customer service, referrals, and satisfaction.
