Job summary

PAM Health Rehabilitation Hospital of Kyle in Kyle, TX is seeking a Clinical Navigator to expand referral networks and support patient admissions. You will cultivate relationships with physicians, discharge planners, and payer representatives to drive referrals and improve the patient journey.

The role reports to the Director of Business Development and requires at least two years in a licensed health field; a health-related degree and licensure are preferred.

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech-language therapy required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts and analyzes referral and admission data from the territory.
• Collaborates with Admissions Manager and staff to facilitate referral conversion.
• Develops and maintains relationships with physicians, discharge planners, and payer representatives.
