Job summary

PruittHealth Corporation is seeking a Registered Nurse (RN) to lead our Home Health services as Clinical Manager. This role focuses on overseeing patient care, coordinating admissions, and guiding clinical staff with compassion and integrity.

You will mentor teams, ensure regulatory compliance, and partner with systems like MatrixCare to optimize workflows and reporting, delivering high-quality care in patients' homes.

Qualifications

• Active, unrestricted RN license in the state of practice.
• Minimum of two years RN experience (preferred).
• At least one year of Home Health experience (preferred).
• Current CPR certification.

Responsibilities

• Provide daily clinical oversight of patient care services.
• Coordinate referrals and admissions with internal and external sources.
• Evaluate patient needs and agency capabilities before admission.
• Supervise care planning, partner assignments, and care delivery.
• Ensure compliance with regulatory standards and agency policies.
• Mentor and coach clinical staff to promote morale and retention.
• Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.
