Job summary

We are seeking an RN Clinical Manager to lead clinical operations and patient care services within PruittHealth @ Home. The role focuses on guiding care teams, ensuring quality outcomes, and collaborating with referrals to support patients at home.

The ideal candidate holds an active RN license, with at least two years of RN experience (preferably in home health) and CPR certification, demonstrating compassionate leadership and patient‑centered care.

Qualifications

• Active, unrestricted RN license in the state of practice.
• Minimum of two years RN experience (preferred).
• At least one year of Home Health experience (preferred).
• Current CPR certification.

Responsibilities

• Provide daily clinical oversight of patient care services.
• Coordinate referrals and admissions with internal and external sources.
• Evaluate patient needs and agency capabilities before admission.
• Supervise care planning, partner assignments, and care delivery.
• Ensure compliance with regulatory standards and agency policies.
• Mentor and coach clinical staff to promote morale and retention.
• Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.
