Job summary

PruittHealth in Columbia, SC seeks an experienced RN to serve as Clinical Manager for Home Health Services. Lead clinical operations, oversee patient care, and ensure quality and compliance across the division.

You will mentor staff, coordinate referrals, and use MatrixCare and SH Partner systems to drive efficient workflows. A valid RN license and CPR certification are required, with preference for Home Health experience.

Qualifications

• Active, unrestricted RN license in the state of practice.
• Minimum of two years RN experience (preferred).
• At least one year of Home Health experience (preferred).
• Current CPR certification.

Responsibilities

• Provide daily clinical oversight of patient care services.
• Coordinate referrals and admissions with internal and external sources.
• Evaluate patient needs and agency capabilities before admission.
• Supervise care planning, partner assignments, and care delivery.
• Ensure compliance with regulatory standards and agency policies.
• Mentor and coach clinical staff to promote morale and retention.
• Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.
