Job summary

PruittHealth @ Home is seeking a Registered Nurse (RN) Clinical Manager to lead clinical operations and patient care in our home health division. This role requires hands-on leadership and a compassionate approach to delivering high-quality home-based care.

You will oversee referrals, admissions, care planning, and staff coaching, while ensuring compliance with regulatory standards and agency policies. Proficiency with MatrixCare and related systems is a plus.

Qualifications

• Active, unrestricted RN license in the state of practice.
• Minimum of two years RN experience (preferred).
• At least one year of Home Health experience (preferred).
• Current CPR certification.

Responsibilities

• Provide daily clinical oversight of patient care services.
• Coordinate referrals and admissions with internal and external sources.
• Evaluate patient needs and agency capabilities before admission.
• Supervise care planning, partner assignments, and care delivery.
• Ensure compliance with regulatory standards and agency policies.
• Mentor and coach clinical staff to promote morale and retention.
• Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.
