Job summary

PruittHealth @ Home is seeking a Registered Nurse (RN) - Clinical Manager to oversee home health clinical operations and patient care in our Jacksonville region. Lead the care team, ensure quality standards, and coordinate referrals and admissions.

Active, unrestricted RN license in Florida, with 2+ years RN experience and 1+ year Home Health experience preferred. CPR certification required. This full-time role offers a compassionate, family-like culture and competitive pay.

Qualifications

• Active, unrestricted RN license in the state of practice (Florida).
• Minimum of two years RN experience (preferred).
• At least one year of Home Health experience (preferred).
• Current CPR certification.

Responsibilities

• Provide daily clinical oversight of patient care services.

• Evaluate patient needs and agency capabilities before admission.
• Supervise care planning, partner assignments, and care delivery.
• Ensure compliance with regulatory standards and agency policies.
