We are currently looking for a Clinical Manager, Homecare to join our team. The Clinical manager is responsible for the day-to-day operations of the Homecare interdisciplinary team as well as providing coordination of internal and external services for patients on the team.
1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers2. Provides daily operational responsibility for the Homecare team and coordination of patient care3. Acts as a resource for field staff as needed4. Monitors quality and appropriateness of services provided to ensure patient satisfaction5. Participates in VNAHG’s QAPI and in-service programs6. Adheres to the organization’s policy in regards to absenteeism and appearance7. Supervises staff in conjunction with the organization’s managerial and leadership practices
1. RN, BSN with licensure in NJ, or Bachelor’s Degree in Physical, Occupational or Speech Therapy2. 2-5 years of community health experience preferred3. Excellent verbal and written communication skills4. Strong organizational and interpersonal skills5. Knowledge of reimbursement and OASIS requirements6. Valid driver’s license
Working Conditions/Physical Demand: Working indoors and outdoors with phone and computer use; clientcustomer contact
