Job summary

Heart’n Soul Hospice is seeking a Clinical Liaison Registered Nurse in Seattle, WA. The role focuses on developing a hospice presence within the healthcare community, performing admissions assessments, and educating physicians and care teams on hospice programs and Medicare/Medicaid reimbursements.

The successful candidate will hold a nursing degree with an active RN license, a valid driver's license, and at least two years of RN experience, preferably in hospice or healthcare settings.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse.
• Valid driver’s license and car insurance.
• Two years of RN experience, hospice/healthcare preferred.

Responsibilities

• Develop Hospice presence in healthcare communities and build partnerships with hospitals and providers.
• Perform comprehensive nursing assessments for hospice admission suitability.
• Educate medical professionals on hospice programs, pain management, and reimbursements.
• Establish public relations to improve understanding of agency services and foster physician/community relationships.
• Lead new business development and support Community Education Representatives as needed.
• Ensure compliance with local, state, and federal regulations and agency policies.

Clinical Liaison Registered Nurse

What You Must Have

What You Will Do

• Develop a Hospice presence in the healthcare community with hospitals and health care providers.
• Performs comprehensive nursing assessment and evaluation for appropriateness of hospice admission.
• Develop, educate and consult with medical professionals concerning hospice programs, pain management and Medicare/Medicaid/commercial reimbursements.
• Establish public relations programs for interpretation of the agency's services and to foster good working relations with physicians and community agencies. Responsible for the implementation of specific projects and initiatives as directed.
• Responsible for new business development and assisting Community Education Representatives with new account development when needed.
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
